Does your child receive help from multiple services?

• Does your child have emotional or behavioral issues in school?
• Has your child been in trouble with law enforcement?
• Is your child at risk for out-of-home placement?
• Is your child at risk for out-of-school placement?
• Does your child live in Beckham, Roger Mills, Custer, Washita or Blaine County?
• Is your child between the ages of 0 and 21 years old?

If you answered yes to two or more of these questions your child can be referred to Great Plains Systems of Care.
